1. The original American death certificate, or a certified copy.
2. A burial transit permit.
3. A letter of non-contagious disease issued by an authorized health institution.
4. A letter of embalming.
5. A letter of sealed casket.
6. The original passport of the deceased (to be cancelled).
7. The Lebanese extract of civil status (Ikhraj Kayd) of the deceased.
8. A self-addressed stamped return envelope.
9. This formality is free of charge.
