- An application form (click here to download)
- The final divorce act issued by the relevant American court, with its official translation to Arabic
- A recent Family Extract of Civil Status
- A recent birth certificate for each spouse
- A power of attorney granting a lawyer or representative in Lebanon the right to follow up on the finalization of the divorce in Lebanon
- A $35 money order (cash is only accepted if applying in person).
