  1. An application form (click here to download)
  2. The final divorce act issued by the relevant American court, with its official translation to Arabic
  3. A recent Family Extract of Civil Status
  4. A recent birth certificate for each spouse
  5. A power of attorney granting a lawyer or representative in Lebanon the right to follow up on the finalization of the divorce in Lebanon
  6. A $35 money order (cash is only accepted if applying in person).
