Registration of Vital Events \ Death

- An application form (click here to download)
- The official American death certificate
- The Extract of Civil Status of the deceased, or his/ her Family Extract, or his/her Lebanese Identification Card
- The Lebanese passport belonging to the deceased. The passport will be cancelled and mailed back
- A return self-addressed stamped envelope.
