Registration of Vital Events \ Death

  1. An application form (click here to download)
  2. The official American death certificate
  3. The Extract of Civil Status of the deceased, or his/ her Family Extract, or his/her Lebanese Identification Card
  4. The Lebanese passport belonging to the deceased. The passport will be cancelled and mailed back
