- An application form (click here to download)
- The official American birth certificate.
- The parents’ Family Extract, or Extracts of Civil Status.
- The parents’ marriage has to be registered prior to the registration of their children. However, if the marriage has not yet been registered, it is possible to register the marriage and birth at the same time.
- A return self-addressed stamped envelope.
