  1. An application form (click here to download)
  2. The official American marriage certificate
  3. A recent Extract of Civil Status (Ikhraj Kaid Ifradi) or a Family Extract (Ikhraj Kaid Aa’ili)
  4. A birth certificate for non-Lebanese spouses (an official translation to Arabic is required if the certificate is neither in English nor in French).
