In order to renew or issue a Lebanese passport, one is required to fill out and sign an application form (obtainable either in person or by mail from the Consulate General's Headquarter), and submit it with the following documents:

1. Lebanese passport if available along with either a Lebanese Identity Card, or a new Extract of Civil Status (Ikhraj Kaid) or a Family Extract of Civil Status (Ikhraj Kaid Aa’ili).
   
2. An information card with the right thumb fingerprint.
   
3. Three recent 2x2 passport-size photos.
   
4. An American proof of residency (such as U.S. passport, Green Card, or a Valid U.S. Visa)
   
5. A $40 money order for a one year validity or renewal, or a $200 money order for a 5 year validity or renewal. 6- A self-addressed, stamped return envelope.

Additionally:

• Information cards may be obtained in person or by mail. For cards sent by mail, a notary public must notarize the right thumb affix on the card.
  
• Application forms along with all required documentation may be sent by mail or submitted in person at the Lebanese diplomatic mission in your area.
  
• Once in Beirut, holders of Lebanese passports from the old specimen (the burgundy color) are strongly urged to replace their passport with a machine readable passport from the new specimen (the ‘blue’ or computerized specimen). Passports from the new specimen can only be issued in Lebanon by the ‘Sûreté Générale’, but are renewable in Lebanese diplomatic missions abroad for either a one year or a five year period. Passports from the old specimen may only be renewed for one year.
  
• For stolen or lost passports, a police report is required, along with a three day announcement in the local newspaper.
