Fill out and stamp your right thumb on an identification card at the Consulate.
If unable to come to the Consulate, you may contact us & give us your mailing address.
We will mail you an identification card. Your right thumb fingerprint should be affixed on the pink card in front of a Notary Public and attach the following statement:

"Before me, a notary public, in and for County of ________________, personally appeared Mr./Mrs.____________________________________, holder of Lebanese Passport or Lebanese ID (Tazkarat Al-Hawiya or Ikhraj Kayd) No._____________________, issued Date:________________________, Place:__________________, who signed and affixed his/her right hand thumb on the annexed identification cards. In testimony whereof, I have hereunto subscribed my name at (County) ______________________________, (date)__________________(Seal)"

• Burgundy Passport

Holders of Lebanese passports from the old specimen (the burgundy color) are strongly urged to replace thier passports with a biometric passport (blue specimen).
The Consulate continues to renew the burgundy passport for a period of one year only.

*Requirements:
Fill out a Passport Application Form and present it to the Consulate with the following documents:

1. Actual passport with 1 photocopy of the first three pages.
   
   
2. Recent Personal civil registration (Ikhraj Kayd Fardy) or the new Lebanese ID (Hawiya) with 2 photocopies.
   
   
3. An American proof of residency (such as U.S. passport, Green Card, or a Valid U.S. Visa)
   
   
4. An information card with the right thumb fingerprint.
   
   
   
5. 2 photos (Passport size).
   
   
6. For applicants who are under 18 years of age, a Consent Letter signed by both parents "Authorizing the child to obtain a passport".
   
   • In person: at the Consulate in front of the Consular Officer.
   • By mail: in front of a notary public.
     
     
7. Self-addressed stamped return envelop return envelope, WE DO NOT ACCEPT REGULAR MAIL!
   
   
8. Fees: $40 either cash or with a money order payable to the Consulate General of Lebanon in Los Angeles.
   
   

• Biometric Blue Passport

Passports of the new specimen (Biometric blue passport) may be renewed for either a one year period or a five year period.

*Requirements:
Fill out a Passport Application Form and present it to the Consulate with the following documents:

1. Actual passport with 1 photocopy of the first page.
   
   
2. Recent (not exceeding 8 years) Personal civil registration (Ikhraj Kayd Fardy) or the new Lebanese ID (Hawiya) with 2 photocopies.
   
   
3. An American proof of residency (such as U.S. passport, Green Card, or a Valid U.S. Visa)
   
   
4. An information card with the right thumb fingerprint.
   
   
5. 2 photos (Passport size).
   
   
6. For applicants under 18 years of age, a Consent Letter signed by both parents "Authorizing the child to obtain a passport".
